Preface

This book provides clinical tips on the management of common emergencies that are regularly faced by critical care and acute care cardiologists in resource-limited settings. Based on the current guidelines, it explores the evaluation of the patient, followed by its treatment methodology. It highlights the beneficial effects of the use of cardiac drugs during an emergency. There is also a special section on preoperative evaluation and postoperative management of cardiac patients of different subsets. Medicolegal/documentation points are also discussed where relevant. It is useful as a ready reference for physicians, anaesthetists and cardiologists.

With the explosive development of interventional cardiology, a change has occurred. We have more critical cases, a larger elderly population and more patients having comorbidities like kidney and lung disease. Intervention in this patient population is riskier, and the post-procedure course requires closer monitoring and deeper interdisciplinary knowledge for better outcomes. In fact, the more difficult or high stakes a procedure is, the more important post-procedure care becomes – as important as the procedure itself – to sail through choppy waters. Knowledge of medical or surgical intensive care strategies may help but may not be enough for cardiac care unit (CCU) needs. Thus, a text is required to bridge the gap between cardiology and intensive care. 

While I was figuring out an arterial blood gas (ABG) in cardiogenic shock for my resident, I realized that there is a dearth of texts in this field. But a voluminous text will not serve this purpose as the physicians busy in the CCU – residents, trainees, physicians with training in intensive care, cardiologists and anaesthetists – hardly have time. A handbook in the form of dialogue or in question-and-answer format delivers the necessary information to-the-point in a lively way. Just as in the bedside, personal perspectives reinforce education – this aspect has also been incorporated in the text, as are pitfalls, real cases and nightmares. The authors who have been selected for their wide experience in managing cardiac intensive care have been gracious enough to subscribe to this format. We hope this book will provide a much-needed guidance to the “CCU warriors” who toil day in and day out to ensure that critical cardiac patients live to see the light of the day
